PHASE 1: ANESTHESIA
07:30 The Polish twin have been in the operation room.
07:45 Olga had her anesthesia with no problems, then Daria was anesthetized too with no complication
08:00 The Team is inserting the arterial line and the central venous lines
09:00 Phase 1 almost completed, the team inserting the epidural catheter for Daria & by this stage the Phase 1 will be completed successfully
09:20 The Phase 1 has been completed successfully without any complications, the Team is doing endoscopy for the shared anal opening to look at the shared part of the rectum
PHASE 2: Preparation and Positioning
10:00 This phase started few minutes ago by planning the incisions site then the preparation and positioning.
PHASE 3: PEDIATRIC SURGERY & PLASTIC SURGERY
10:40 Skin incision was made, followed by underling tissue, the incision was carried between 2 vaginae, and then the peritonium was opened.
11:45 The crossing vasels has been identified and clamped.
PHASE 4: PEDIATRIC SURGERY
12:00 The crossing vasels has been identified and clamped.
12:15 All lower limbs are viable with good oxygenation as it shown clinical and by pulse oxymeter .The vasels has been cut successfully.
13:50 The dissection of the intestine as well as the anus is done, and the urological system.
14:45 this stage had been completed successfully with no complications. The Team will now flip the Twin to the other side to start the next phase.
PHASE 5: PEDIATRIC SURGERY & NEUROSURGERY (Repositioning & Resterilization)
15:45 The twin had been flipped to the other side and the Resterilization had been done successfully.The site of the incisions had been marked as well
PHASE 6: NEUROSURGERY (Spinal Canal)
16:30 The Team started the separation of the spinal cord, this phase will take approximately 3-4 hours.
19:30 There have been a good progress in this phase, and the Team is now in the final stage of the separation of the spinal cord.
21.30 The spinal cord had been separated successfully, all nerves were not injured. The “dura” (the layer that covers the spinal cord) the Team will use special glue to cover to seal the spinal cord
PHASE 7: PEDIATRIC ORTHOPEDICS Pelvis Separation
21:45 The Team has started the separation of the lower part of the back bones “sacram”.
PHASE 8:
22:30 SEPARATION OF THE TWINS TO (2) TABLES – A & B, where the wounds are being closed.
22:20 The Polish twin have been successfully separated .
01:45 The separation surgery for the Polish conjoined twin “Daria & Olga” have been completed successfully. The two girls have been transported to the PICU.
